Health & Science after SPM: the work, the routes in and how to try it

What people in health and science jobs do each week, the routes in after SPM (Health Ministry diplomas, university diplomas, matrikulasi or Asasi to a degree), which jobs need a licence from a regulator, and how to try the work now.

Health & Science goes well beyond doctor or nurse. Behind every ward are labs, scans, pharmacies, rehabilitation gyms and public-health teams, and outside hospitals there are science jobs in testing labs, factories and research. This guide shows what the work looks like in a week, the routes in after SPM with what each one asked for, which jobs need a licence before you can work, and how to try the work now.

For named programmes, the institutions that run them and who funds each route, see Where to study Health & Science in Malaysia, and who funds it.

Key facts

  • The Health Ministry trains nurses, assistant medical officers, lab technologists, physiotherapists and six other health workers in three-year diplomas, applied for through SPA, the Public Service Commission.[1][2]
  • School leavers applying for that training must have an SPM credit in Bahasa Melayu, be at least 18 when applications close and no older than 25 when the course starts.[1]
  • Allied health workers such as physiotherapists, lab technologists and radiographers must be registered and hold a practising certificate by 31 December 2026; working without them after that is an offence.[3][4]
  • New pharmacists work at least one year under provisional registration before full registration.[5]
  • Doctors must serve at least three years in the public services after full registration.[6]

What the work actually is

A job title tells you little about the week. Look at the tasks instead (Tasks, not job titles shows how). Rows with a source follow the allied health council's own profession descriptions; the others are typical weeks. Either way, ask a practitioner how theirs compares.

RoleWhat a week can look like
Staff nurse (jururawat)Take handover at the start of a shift, check patients' vital signs, give medicines on time, dress wounds, update records and explain to a worried family what happens next
Assistant medical officer (penolong pegawai perubatan)See patients in an emergency department or clinic, do the first assessment and treatment, and decide who needs a doctor now
Medical laboratory technologistRun routine and special tests on blood and tissue samples, by hand and on automated machines, and make sure every result is reliable before a doctor uses it[4]
Medical laboratory scientist (biomedical scientist)Do the complex analyses, interpret results, set the lab's quality standards, develop new methods and do research[4]
PhysiotherapistAssess how someone moves after an injury, operation or illness, plan exercises, coach them through each session and track progress[4]
Diagnostic radiographerPosition patients, take X-ray and other medical images good enough to diagnose from, and protect everyone from unnecessary radiation[4]
PharmacistCheck prescriptions, prepare and supply medicines, catch drug interactions and explain to patients how to take what they are given
Environmental health officerInspect food premises, control mosquitoes and other disease carriers, test drinking water, help investigate outbreaks and enforce public-health law[4]
DietitianAssess a patient's diet problems and turn nutrition science into a plan they can follow, using medical nutrition therapy[4]
Lab analyst or quality-control scientist (outside healthcare)Test food, water, medicines or products against standards, record every reading and write the report. Only chemists registered with Institut Kimia Malaysia may sign a report of analysis meant for the public[7]

Many of these jobs start with a diploma, not a degree. The Health Ministry's own training covers nursing, assistant medical officer, assistant environmental health officer, X-ray, physiotherapy, occupational therapy, medical laboratory technology, assistant pharmacist, dental technology and dental therapy.[1] Pharmacists, doctors, scientists and dietitians usually need a degree.

Routes in after SPM

Real examples from official pages. The SPM rules are for 2026/27; check them again for your intake.

RouteExampleWhat it asked forHow long
Health Ministry diplomaLatihan Jururawat, Juruteknologi Makmal Perubatan, Jurupulih Perubatan Fisioterapi and seven more, at Health Ministry training institutes (ILKKM), applied for through SPA[2]Malaysian citizen, SPM with a credit in Bahasa Melayu, plus each programme's own minimum; at least 18 when applications close and no older than 25 at the start[1]3 years[1]
University diplomaHealth diplomas listed in e-Panduan UPUOnline3 SPM credits including Bahasa Melayu, and a pass in Sejarah, plus each programme's special requirements[8]See Diplomas
Matrikulasi or Asasi, then a degreeUSM Bachelor of Medical Radiation (Hons)A matrikulasi or Asasi CGPA of 3.00, at least C+ in two of Biology, Chemistry and Physics or Mathematics, MUET Band 3.0 and a strong interview[9]8 semesters for the degree[9]
Diploma, then a degreeThe same USM degreeA related diploma with a CGPA of at least 2.75[9]8 semesters; some courses may be exempted[9]

A few things the table does not show:

  • Intake dates are moving. SPA's page says Health Ministry training now takes students in April and October, from the October 2026 intake, but its FAQ still says January and July.[1] Check the page in the month you plan to apply.
  • Degree routes are competitive. The USM example asks for strong science grades, a MUET band above the university's general minimum, and an interview.[9] Put a second route on your list, such as a diploma that can lead to the same degree later.
  • Form 6 also leads to health and science degrees. See Form 6 (STPM), Matrikulasi and Asasi and foundation for their entry rules, and Routes after SPM, side by side to compare them.
  • TVET skills courses lead to many careers, but the clinical jobs above are licensed, so they need the diploma or degree the regulator recognises. TVET after SPM covers what skills routes offer.

Which jobs need a licence

Most clinical health jobs are regulated by law. That protects patients, and it means the course you choose has to be one the regulator accepts.

  • Nurses are registered with the Nursing Board Malaysia (Lembaga Jururawat Malaysia). After training, your registration certificate is collected from your university or college, and each year your employer applies for your annual practising certificate.[10] The Board tells you to check a college's nursing programme on the MQA register.[10]
  • Allied health workers (including medical laboratory technologists and scientists, physiotherapists, radiographers, dietitians and environmental health officers) come under the Allied Health Professions Act 2016. The Malaysian Allied Health Professions Council (MPKBM) set 31 December 2026 as the deadline to register and get a practising certificate. After that, practising without both is an offence.[3][4]
  • Pharmacists are first provisionally registered and work at least a year as a pharmacist in approved training premises, then apply for full registration.[5][11] Once fully registered, the Director General of Health can require you to serve in the public service.[5]
  • Doctors get provisional registration for supervised training as a house officer, with four-month postings in six departments, then full registration.[12][6] After that they must serve at least three years in the public services.[6]
  • Chemists who sign reports of analysis for the public must be registered with Institut Kimia Malaysia.[7]

Before you accept a place at a private college, look up the exact programme on the Malaysian Qualifications Register(opens in a new tab) and ask the college in writing whether its graduates can register with the board for the job you want.

The money, briefly

First pay depends on your route and, in government posts, on a published salary grade, and the time it takes to register can delay your first pay packet. The money in Health & Science has the latest first-pay figures by certificate, diploma and degree, and First salaries by career area compares all eight areas.

How AI is changing it

AI is moving into the reading, recording and checking side of health work, such as screening images and coding records, faster than into hands-on care. AI and the future of Health and Science work sets out the evidence task by task and keeps what has been observed apart from what is only projected.

Try it now

What this means for you

  • Look past doctor and nurse. Labs, imaging, rehabilitation, pharmacy and public health each mix patients, machines and data differently, so pick by the tasks you enjoy.
  • Many health careers start with a three-year diploma, and the Health Ministry runs its own. Check the age window: 18 when applications close and no older than 25 at the start.
  • A licence is part of the job. Choose a programme the regulator recognises, or you may finish the course and still be unable to work in that role.
  • Some routes come with years of service: doctors serve at least three years in the public services, and pharmacists can be required to.
  • Rules are changing right now, from the allied health registration deadline to SPA's intake months. Treat this guide as a starting point and check each page before you apply.

Your next step

  1. Open SPA's Latihan Separa Perubatan page(opens in a new tab), pick two of the Health Ministry diplomas, and note each one's minimum SPM results and the next application window.
  2. On Day 2, ask a practitioner: "Which registration did you need before your first day, and how long did it take?" Write their answer in your Future Map.

Choose one course above. Before your 30-day check-in, try the first section and save one learning example in Future Map. At your 90-day check-in, decide whether to continue or switch. Read the course guide for access, age and availability notes.
